  • Where is the office located?

    Tulsa Clinic is located at 2000 S. Wheeling Avenue, Ste 510 in the Williams Medical Plaza which is on the corner of 21st and Wheeling on the St. John Medical Center campus.


    Muskogee Clinic is located in the Fresenius Dialysis Unit at 3312 Okmulgee Avenue.


    Bartlesville Clinic is located at 3500 State Street in the Doctor’s Building which is north of the Emergency Room at Jane Phillips Medical Center.


    Our Independence Office is located in the Ascension St. John Clinic Building at 800 W. Laurel Street.

  • What do I need to bring to my appointment?

    Please bring your insurance cards, a photo ID, and a current list of medications. In addition, if this is your 1st appointment, please complete and bring with you the New Patient forms that we send you.

  • When do I pay?

    Payment is expected upon check-in. We accept Cash, Check, Visa, Mastercard and Discover.

  • How do I get a referral?

    New patients must be referred to our office by their primary care physician or other physician.

  • What are the office hours?

    Tulsa Clinic is open Monday - Friday 8:30 - 4:30

    Closed 12:00 - 1:00 for Lunch

    Closed 2nd and 4th Fridays at noon


    Muskogee Clinic is open Monday-Wednesday-Friday, 8:30 – 4:30

    Closed 12:00 -1:00 for Lunch


    The Bartlesville office is open Monday-Wednesday-Friday 8:30 – 4:30

    Closed 12:00 -1:00 for Lunch


    The Independence Office is open the 1st and 3rd Tuesday of each month. 9:00 – 4:00

    Closed 12:00 – 1:00 for Lunch


  • I need my medicine refilled.

    Please call your pharmacy for your prescription refills.

  • What do I need to do to obtain a copy of my medical records?

    You will need to sign a release in order to obtain your medical records. Releases are available at the clinics.

  • Should I bring someone with me?

    On your 1st visit, it may be helpful to have someone with you to listen to the information that is given to you.

  • What is Nephrology and why was I referred to you?

    Nephrology is the study and/or treatment of the kidney as it relates to kidney disease. You may have been referred to us because of a decrease in kidney function, elevated and/or uncontrolled hypertension, or protein in your urine.

  • What hospitals do you see patients in?

    We see patients at Ascension St. John Medical Center in Tulsa, OK; Jane Phillips Medical Center in Bartlesville, OK; St. Francis Muskogee in Muskogee, OK.
